Closest near misses you've experienced...

This is a non-football thread:

This afternoon I was on my way home when I witnessed a step ladder fall of the top of a rather large white work van. Looking it was at a crossroads and fell into the (thank god empty) crossing walkway, I can imagine the air being blue in the cars immediately behind (I was at the other lane) because if it had landed on another car or he'd have had someone in front it could have had the window through and the driver could have been sued and facing an expensive legal settlement or even prison (for recklessness occasioning GBH or worst involuntary manslaughter).

So this made me wonder-what is the biggest near miss catastrophe you guys have witnessed?
