(no idea about the answer!)
Chelsea was definitely not the first. I remember a CC first round tie 70/71 between Mönchengladbach and Everton that Everton won on penalties.

I don't remember any other but there might be some. The "unbeatability" of German teams on penalties is just a modern myth.

Correct.
IK Sleipner won the Allsvenskan in 1931 and now currently play in level 3.

Nothing to do with Ajax, a respectable and historical team.

USA? I guess the structure of the MLS might give a championship with a negative score.
DC United?

No Brazil.
Back before Brazil had a normal national championship.

Australia is a country that crossed my mind, too.
Have no idea, but here are some other options:
Mexico, South Africa, Senegal.
Interesting question.

I woud add to the list of guesses:
- San Marino (very convoluted play-off system defining the league champion);
- Mexico (also a mixed system with groups and play-off)

San Marino is wrong, as there is no European country above the correct ones.
Mexico not correct, too, as I mentioned it before and
Di_Gule has already said that is wrong.
